Two things:

1) Legitimate sites occasionally get hacked.  Just because you don't go 
to less than reputable sites doesn't mean you can't run into problems.

2) If you connect, even briefly, you may be infected.

As Alan has pointed out, AV and anti-spy/malware software don't catch 
them all and never will.  Thinking you're safe is a recipe for disaster 
- "Cockiness is that feeling you have just before you know better."

Two things (again):

1) The XP firewall will only block incoming attempts, hardly a complete 
solution to monitoring network access.  Even the better firewalls can be 
fooled.

2) I'll wager that program sitting on your shelf is not one of the 
better ones if it supports OSes that old.  Not that those OSes should be 
ignored but the best ones seem to put their eggs in the newer OSes' 
baskets.  And, as well, if you really think it caught everything, you 
need to revisit the latest analyses of how the various malware and the 
anti-malware software work.  Nothing catches all of them but some are 
better than others.
