When FDR died in April of 1945, Truman was sworn in as "Acting
President".  It was not until 1967, due to an executive order by
LBJ, that a VP assumed the office of President upon the death or
incapacity of the President.  Of course, that means Ronald Reagan
was no longer President when he underwent surgery following the
assassination attempt by John Hinckley...

Truman was not elected President until 1948.  In 1952, Truman chose
not to run for re-election.

The 22nd Amendment did not limit the number of terms that Truman could
have been elected to, or serve.  Nor does the 22nd Amendment limit the
number of terms that any other President can be elected to, or serve.
However, with the exception of Truman, no President can be elected or
serve for more than two consecutive terms.  IOW, Bill Clinton can be
elected to, and serve, another first term, and a subesequent second
term.  And so can George W. Bush.  Except that George W. Bush will
have to sit out a while before being elected to another first term.
But then, there is nothing stopping George W. Bush from running for
Vice President, with Condi Rice being his running mate.  And then
Condi can resign from office, allowing George W. Bush to become
President...
